  • Correlation between apolipoprotein b/a1 and the risk of metabolic-related fatty liver disease depending on the lipid profile (Issue № 1, 2025)

    Abstract

    Background. The aim of this study was to investigate the relationship between the Apolipoprotein B/Apolipoprotein A1 ratio and the risk of metabolically associated fatty liver disease, considering lipid profile variations and determine whether the Apolipoprotein B/Apolipoprotein A1 ratio can serve as an independent predictor of metabolically associated fatty liver disease and its associated cardiovascular risks.

    Methods. A total of 377 patients diagnosed with cardiovascular disease and coexisting metabolically associated fatty liver disease were stratified into high-risk and very high-risk groups based on established clinical and imaging criteria. Lipid profiles, Apolipoprotein B, and Apolipoprotein A1 levels were measured, and their associations with cardiovascular and metabolic parameters were analyzed using correlation and regression models.

    Results. The findings reveal a strong positive correlation between Apolipoprotein B levels and atherogenic lipid markers such as low-density lipoproteins cholesterol and total cholesterol, as well as a moderate correlation with triglycerides. In contrast, Apolipoprotein A1 demonstrated a strong positive association with high-density lipoproteins cholesterol and an inverse correlation with metabolic indicators such as Hemoglobin A1c and glucose, suggesting a potential protective role. The Apolipoprotein B/Apolipoprotein A1 ratio emerged as a more accurate predictor of cardiovascular and metabolic risk than traditional lipid ratios. Furthermore, the Apolipoprotein B/Apolipoprotein A1 ratio was significantly associated with the presence and severity of metabolically associated fatty liver disease, reinforcing its potential utility as a biomarker for metabolic liver disease.

    Conclusion. This study highlights the clinical relevance of incorporating apolipoprotein measurements into the routine assessment of individuals at risk for metabolic and cardiovascular disease.

  • Single-center experience of heart transplantation at the heart center of Astana (Issue № 3, 2024)

    Abstract

    Background. Chronic heart failure is a major global health issue. As a complication of most cardiovascular diseases, it affects 4% of the population. Heart transplantation is the gold standard in the treatment of patients with end-stage heart failure. Objective of the study was to evaluate early and long-term outcomes of heart transplants performed at the Heart Center University Medical Center (Heart Center) over a 10-year period.

    Materials and methods. Cross-sectional study was conducted from 2012 to 2022, 86 orthotopic heart transplants were performed at the Heart Center. The analysis of the obtained results was conducted retrospectively.

    Results. From August 2012 to December 2022, 114 patients were on the waiting list for heart transplantation. Of these, 86 (75.4%) patients underwent transplantation; 10 (8.7%) patients were excluded. Among the 86 patients, 49 (56.9%) had previously undergone cardiac surgery. Of these, 42 (48.8%) had a left ventricular assist device implanted earlier, 3 (3.4%) had a fully artificial heart, and 2 (2.5%) were on temporary mechanical support (central veno-arterial Extracorporeal membrane oxygenation). Hospital mortality was 8 (9.3%) recipients. In 2.7% of cases, the cause of death was an acute cerebrovascular accident on the second day post-surgery. Postoperative renal dysfunction was noted in 28 (32.5%) patients. An analysis of all performed heart transplantation cases showed a 30-day survival rate of 94%, a 1-year survival rate of 84.3%, and a 5-year survival rate of 64.7%.

    Conclusion. In the hospital period and the first 6 months after heart transplantation, infectious-septic complications were predominant, whereas in later periods, rejection reactions were more common.

  • FEATURES OF THE COURSE OF NON-ALCOHOLIC FATTY LIVER DISEASE INTYPE 2 DIABETES MELLITUS AND IMPAIRED GLUCOSE TOLERANCE (Issue № 2, 2024)

    Abstract

    Background. Non-alcoholic fatty liver disease and type 2 diabetes mellitus or impaired glucose tolerance are common diseases with a high risk of developing cardiovascular diseases, the leading cause of disability and death. Our aim is to develop new methods for screening, prevention, and treatment of cardiovascular diseases in patients with non-alcoholic fatty liver disease, type 2 diabetes mellitus, and impaired glucose tolerance.

    Methods and methods. This study is single-center, open-label, uncontrolled, diagnostic study. Between 2023 and February 2024, 216 patients with cardiovascular diseases and concomitant non-alcoholic fatty liver disease, type 2 diabetes, and impaired glucose tolerance were selected at Heart Center “University Medical Center” Corporate Fund.

    Results. All examined patients with cardiovascular diseases and concomitant non-alcoholic fatty liver disease showed increased liver enzyme activity. Blood lipid profile indicators were significantly higher than optimal levels for patients with cardiovascular diseases. There was also a significant increase in liver enzymes, C-reactive protein, triglycerides, and lipoproteins in patients with cardiovascular diseases and non-alcoholic fatty liver disease combined with type 2 diabetes and impaired glucose tolerance.

    Conclusion. Liver enzyme activity, C-reactive protein, glucose, and lipid profile analysis showed significant increases in these indicators in patients with cardiovascular diseases and non-alcoholic fatty liver disease, especially in the presence of type 2 diabetes and impaired glucose tolerance. All examined patients showed increased liver enzyme activity and lipid levels, indicating the impact of these diseases on the liver and metabolism.

    Abstract

    Background. Currently, in Kazakhstan, the issue of chronic heart failure is becoming increasingly relevant, with high mortality from the terminal stage of chronic heart failure, especially in patients with III-IV functional class. Heart transplantation represents the "gold" standard of surgical treatment for terminal chronic heart failure, but endomyocardial biopsy, used for monitoring the transplanted heart, is an invasive and inconvenient procedure. Aim of this study is to generate data which can aid in more precise antibody-mediated rejection diagnosis, assisting in distinguishing between antibody-mediated rejectionand acute cellular rejection and helping us determine the appropriate treatment strategy Materials and

    Methods. This article explores the potential of safe and accurate monitoring of acute transplant rejection using circulating donor-derived cell-free DNA(dd-cfDNA). The study included 40 patients who underwent heart transplantation.

    Results. It was found that 60% of them had a repeat operation, while 40% had a primary one. Various cardiomyopathies, predominantly dilated and ischemic, were the cause of terminal chronic heart failure. The donor-derived cell-free DNA method demonstrates potential in differentiating T-cell-mediated and antibody-mediated rejection, with different patterns donor-derived cell-free DNA elevation. These differences have high clinical significance for diagnosis and treatment tactics.

    Conclusion. Despite the prospects of using donor-derived cell-free DNA, further research is needed to establish threshold values and confirm its effectiveness in clinical practice.
